I believe the last 3 are from what they call a "rolling" shutter that a lot of the newer digital devices use. The "shutter" rolls across the aperture exposing the cmos a line at a time instead of all at once like the old school shutters do.

I have seen photos of the effect, but this is the first video I have seen of it... pretty wild!
