Fail, as used on the internet, originated before 4chan's time. Your best bet is probably about 10-15 years ago when multiplayer 3D FPSs became common. IIRC Unreal Tournament said something along the lines of "epic fail" if you committed suicide (just like BF:BC2 does).
...I'm getting tired of not being able to figure this crap out because it's apparently made for computer-illiterate people by computer-illiterate people. lol
