Wallpaper? It looks like a complete windows 7 mod.
I'd guess a combo of wallpaper, cosmetic personal preferences, office toolbar, and a sidebar gadget or two.
My mind says Technic, but my body says Duplo.

Just go play with gadgets, or code your own. There's probably lots of them already out there which can do much the same stuff that's on your screenshot and look even better.
Your screenshot is evidently taken from a PC used for lots of gaming, gamers are often nerds who treat their computers as toys and spend hours tweaking trivial cosmetics.
Looks like the taskbar and systray were moved to the right edge. Except for the wallpaper image, the digital clock thing (which might be a standard gadget), and the font (which might be a standard font I just never use), everything I see on that image is already possible through fiddling with personalization and settings and options in plain vanilla Win7. A lot of fine control is available.

Ah thanks for that reply, helpful. Just what I needed.
However, I don't really know how to code my own mods and such. I wouldn't even know where to start for looking for gadgets, tried google - failed horribly.
